See doctor and attorney first.

Submit application that will be deny waste of time.

Agree.

IMO OP should probably find an attorney first and then discuss the path forward for the doctor.

The attorney could help OP clearly state the reasons for denial and help the doctor and court better understand OP.

your best bet probably would be to seek expungement of all of the violations.

Can’t happen.  You can’t expunge more than one crime and three disorderly persons offenses in this state.  Those are pretty generous limits, in my opinion. https://www.njcourts.gov/selfhelp/expungement.html

While the OP is undoubtedly a troll, denying permits to repeat offenders is how the system is supposed to work.

Why do you need a letter from a doctor?

Lack of grammar and the ability to spell in the English language is not a prerequisite for a FPID or P2P, but a clean criminal record is! Doctors are NOT the answer here......A lawyer is!
