New ... New Jersey Universal Fingerprint Form

Some municipal police departments are not aware that there is a new NJ Universal Fingerprint Form.  If the properly completed form does not have the form code on the bottom ...  IDG_NJAPP_020115_V ... it will be rejected by the finger printing companies.

Here's what the form looks like - though for a different fingerprinting reason ...
