Medical Insurance Spousal Surcharge

Asking for a friend ... many companies are now charging a "spousal surcharge" if you choose to include your spouse on your plan when the spouse is eligible for medical coverage through their own employer.  Unless you self-report this, how would your employer know that your spouse is eligible for other coverage?  What would possibly be the downside of not reporting?

Technically, if you do not disclose the information, the insurance company can deny coverage when your spouse needs it.

It would be fraud.  It's a bitch to pay more, but it's really not worth it.  Insurance companies are really good at making sure everything is true and will deny coverage if they find out you were less than truthful.
